Arytenoid-type laryngomalacia treated by suturing excess mucosa

Background: Laser resection of excess mucosa is effective for the treatment of arytenoid-type laryngomalacia. However, postoperative complications have been reported, including supraglottic stenosis and granulation due to interarytenoid adhesion. This paper reports two cases of acquired arytenoid-type laryngomalacia that were treated by suturing the excess mucosa of both arytenoids.

Case presentation: The primary disease was encepha-lopathy due to status epilepticus in Case 1 and was hypoxicischemic encephalopathy during surgery for congenital heart disease in Case 2. Long-term intubation and an indwelling nasogastric tube contributed to the development of laryngomalacia. Both patients demonstrated good progress after surgery without post-operative complications.

Conclusions: This surgical method is minimally invasive to the larynx and is effective in preventing supraglottic stenosis, which can be caused by granulation in cases where chronic stimulation of the arytenoids is expected, such as cases with a continuous indwelling nasogastric tube.
